Waiver Agreement (Adult over 18)

1. CONSENT TO INSTRUCTION

I understand that classes may be taught by the head instructor, qualified instructors, junior instructors, senior students, or guest instructors, at the school's discretion. I acknowledge that I may not always be able to train with the instructor of my choice. Regardless of who is teaching, I agree to follow school etiquette and show respect to all instructors.

I understand that I am responsible for my own safety at all times, and I consent to be instructed by any instructor the school deems qualified. This agreement and waiver apply regardless of who is teaching the class.


2. NOTICE OF PHYSICAL CONTACT

Martial arts training includes physical contact. Any part of the body may be contacted during regular practice and drills, including hands, legs, chest, back, buttocks, or groin. Contact may occur intentionally as part of training or incidentally through movement.

When adults, minors, males, and females train together, the school's purpose is to provide a safe and respectful training environment. Students must conduct themselves appropriately at all times.

If a student feels uncomfortable with an exercise or training partner, or if any contact seems inappropriate, excessive, or beyond the scope of training, the student may immediately stop the exercise and inform the instructor privately. If any conduct appears unsafe or criminal, the instructor and/or authorities may be notified.


3. CONSENT TO PHYSICAL CONTACT

I understand the nature of physical contact in martial arts training, and I consent to such contact for instructional and training purposes. I know I may immediately remove myself from any drill or exercise that seems unsafe or makes me uncom


4. ADVISORY OF RIGHTS AND RESPONSIBILITIES

Safety is the shared responsibility of instructors, staff, and students. All students must evaluate their physical condition, skill level, and limitations during any exercise. Students have the right and responsibility to excuse themselves from any training they believe may be harmful.

If an instructor gives a direction that seems unsafe for a student, the student must inform the instructor. Students must ensure they have enough space to train safely, primarily when weapons or partner drills are being performed.

In the event of an injury or potential injury, students must stop training immediately and inform the instructor. Any person may prevent an exercise if it appears unsafe. If a student notices a hazardous condition (equipment, hazard on the floor, improper training, unsafe conduct), they must either correct it if possible or notify the instructor.


5. ASSUMPTION OF RISK

Martial arts is a physically demanding and potentially dangerous activity. Minor injuries such as bumps, bruises, sprains, strains, and soreness are common. More serious injuries, including but not limited to fractures, tears, concussions, or other injuries, are possible.

Although the school takes reasonable precautions, serious injury, disability, or death may occur. I understand and accept all risks associated with martial arts training and assume full responsibility for my own safety (or my child’s safety if a minor).

By participating, I release All Fitness Kent, instructors, staff, guest instructors, volunteers, students, landlords, and property owners from any liability for injury or harm, except in cases of intentional criminal conduct.

Risk Agreement

AUTHORITY TO TREAT

I, the undersigned, give the instructors, staff, and responsible adults the authority to obtain medical or emergency treatment for the person listed below under “Student Name,” subject to any limitations written below. If I am not the person listed, I am the parent or legal guardian and have the legal right to grant this authority.

Treatment may be provided without prior contact or consent from me or any parent/guardian if necessary and if it does not conflict with any limitations stated below. This authorization begins on the date signed and remains in effect indefinitely.
